Medicaid/CHIP Patients: When you fill out your new enrollment forms, you must name a Main Dentist or Main Dental Home.
To do that, find your nearest office below and pick Dr. Harry Blanek , DDS , MS as your Main Dentist or Main Dental Home.
Please be prepared to provide your Medicaid DMO with your offices phone number which is listed below. Please call your nearest office for any questions.
