Insurance/Financial
Traditional Insurance:
If you have insurance through your employer, when calling to schedule your appointment, please provide the information needed to verify your benefits. We will need the employee's name, the employee's social security number or identification number. We will also need the employee's date of birth, the patient's name, the patient's date of birth and a current phone number. Please understand that filing insurance is a courtesy that we provide free of charge for our patients. We cannot guarantee what your insurance will pay. We will do our best to estimate your co-pay when your treatment plan is presented to you. After 30 days, if an insurance carrier fails to pay a claim, the balance becomes the responsibility of the patient or guardian of the patient.

Medicaid & Tx CHIP:
We accept Medicaid [under 21] and Tx CHIP. When you call to schedule your appointment, please advise the receptionist if you or your child have either Medicaid or Tx CHIP. If you have Medicaid, you are required to bring your current Medicaid paper with you to your appointment. We cannot see you or your child without the current Medicaid paper. If you or your child has Tx CHIP, please provide the information to the receptionist, so that we may verify which tier you are on.

The Smile Plan Discount Program:
For our patients who do not have insurance coverage, we offer a discount plan. The discount plan is good for an entire year and covers every type of dental treatment that our office performs except for orthodontics. The Smile Plan offers discounts up to 55% off. Capital One Dental Financing:
Financing through Capital One is also available to help with the cost of your dental treatment. We can help you apply here in the office. They offer no interest plans as well as plans where the term is offered in payments up to 60 months.
